We support people managing diabetes, COPD, congestive heart failure, infections, injuries, and recovery following surgery. Care may include wound treatment, medication management, nutritional education, and preventative or rehabilitative nursing and therapy. Private nursing, therapy, and caregiver services are also available.

Patient Care Advocates is Medicare certified and licensed by the State of Arizona. Medicare home health coverage depends on eligibility, including homebound status and the need for qualifying skilled services ordered by your provider. Call us to review your circumstances and coverage.
